Choosing the Right Pen Needle
The optimal needle depends on individual factors. Needle length: 4mm is recommended for most adults and children according to current injection guidelines; 5–6mm for standard use; 8mm only for specific body types or injection sites. Gauge: 32G (thinnest) offers the least pain, ideal for sensitive patients; 31G provides a good balance of comfort and flow rate. For a detailed comparison, read our Pen Needles: Complete Comparison & Buying Guide.
Also Compatible with GLP-1 Pens
All our pen needles fit universal pen adapters and are compatible with GLP-1 receptor agonist pens (Ozempic, Trulicity, Mounjaro, Wegovy, Victoza). Check with your prescriber for recommended needle length for your specific medication.

Injection Technique – How to Inject Correctly with Pen Needles
Proper injection technique is essential for effective insulin or GLP-1 therapy and helps minimize discomfort. Follow these steps for a safe and reliable subcutaneous injection:
- Prepare the site: Choose a clean injection area – the abdomen (at least 5 cm from the navel), outer thighs, or upper arms are recommended. Clean the skin if necessary and let it dry.
- Attach the needle: Remove the outer cap of a new, sterile pen needle and screw or click it onto your insulin pen. Remove the inner needle cap just before injecting.
- Prime the pen: Dial 1–2 units and press the plunger until a drop appears at the needle tip. This removes air bubbles and ensures accurate dosing.
- Inject at the correct angle: With 4–5 mm needles, insert the needle straight at 90° without pinching the skin. With 6–8 mm needles, gently pinch a skin fold and insert at 90°, or inject at a 45° angle without a skin fold.
- Deliver the dose: Press the plunger fully and count slowly to 10 before withdrawing the needle. This ensures the complete dose is delivered and reduces leakage at the injection site.
- Withdraw and dispose: Pull the needle out smoothly and dispose of it safely in a sharps container. Do not recap the inner cap.
Rotate your injection sites: Never inject in the same spot twice in a row. Systematic rotation – for example, moving clockwise around the abdomen or alternating between left and right thigh – prevents tissue damage and ensures consistent insulin absorption.
Proper Pen Needle Disposal – Safety First
Used pen needles are classified as sharps waste and must never be thrown into regular household rubbish or recycling bins. An exposed needle poses a serious injury and infection risk to family members, children, and waste collection workers.
Always use a sharps container: Place the outer needle cap back on after use (using the one-hand scoop technique), unscrew the needle from the pen, and drop it into a puncture-resistant sharps container. Purpose-built containers are available from pharmacies and our accessories range.
Pharmacy take-back programmes: In Germany, most pharmacies accept full sharps containers free of charge. Some municipalities also provide designated collection points for medical sharps waste. Never dispose of loose needles in glass jars, plastic bottles, or bags – only certified sharps containers guarantee safe handling throughout the disposal chain.
By disposing of pen needles responsibly, you protect those around you and comply with local waste regulations.
Pen Needles and Lipodystrophy – Why Regular Needle Changes Matter
Lipodystrophy refers to abnormal changes in the fatty tissue beneath the skin – either thickening (lipohypertrophy) or thinning (lipoatrophy) – that can develop at frequently used injection sites. These lumps and indentations are not merely cosmetic: injecting into affected tissue leads to unpredictable insulin absorption, making blood sugar control significantly harder.
One of the primary causes of lipodystrophy is reusing pen needles. After a single use, the ultra-fine needle tip becomes bent and develops microscopic barbs. These damaged tips cause greater tissue trauma with each subsequent injection, promoting scar tissue formation. The golden rule is: one needle – one injection.
In addition to using a fresh needle every time, always rotate your injection sites systematically. If you notice any lumps, hardening, or changes in the skin at your injection sites, consult your diabetes care team. Switching to a fresh area and consistently using new needles can allow affected tissue to recover over time.
Pen Needles for Children and Adolescents with Diabetes
Children and teenagers have thinner subcutaneous fat layers than adults, so choosing the right pen needle length and gauge is especially important. Medical guidelines consistently recommend 4 mm pen needles for paediatric patients of all ages and body types. The short length ensures the injection reaches the subcutaneous tissue without risking intramuscular delivery, which can cause pain and erratic insulin absorption.
For the least painful experience, look for ultra-fine 32G needles such as the BD Ultra-Fine PRO 4 mm (32G) or the BD Micro-Fine+ 4 mm available in our collection. The thinner the gauge number is higher, the finer the needle – 32G needles are among the thinnest available and can make a real difference for children who are anxious about injections.
Tips for parents:
- Let your child choose the injection site from the approved areas to give them a sense of control.
- Use distraction techniques (conversation, music, or a favourite toy) during injection.
- Always use a fresh needle – reused needles hurt more due to tip deformation.
- Praise and positive reinforcement build long-term confidence with self-injection.
Storage and Shelf Life of Pen Needles
Pen needles should be stored at room temperature in a clean, dry place, away from direct sunlight and moisture. There is no need to refrigerate them. Keep the needles in their original, sealed packaging until use – the sterile blister or cap protects against contamination.
Always check the expiry date printed on the box and individual packaging before use. Do not use pen needles past their expiration date, as the sterility of the packaging can no longer be guaranteed. When stored correctly, unused pen needles typically remain sterile and effective for several years from the date of manufacture.

Frequently Asked Questions
What pen needle size is right for me?
For most adults, a 4 mm or 5 mm pen needle at 31G or 32G is recommended by current medical guidelines, regardless of body mass index. Shorter needles (4 mm) are the safest choice because they reliably reach the subcutaneous fat layer without risking intramuscular injection. If you are unsure, consult your diabetes care team for a personalised recommendation.
How often should I change my pen needle?
You should use a new pen needle for every single injection. Reusing needles causes the ultra-fine tip to bend and develop microscopic barbs, which increases pain and tissue damage. Repeated use also raises the risk of lipodystrophy – hardened lumps under the skin that impair insulin absorption.
Are all pen needles universally compatible?
Yes, virtually all pen needles on the market – including BD Ultra-Fine, NovoFine, and BRP Fine – use the universal ISO standard thread and fit all major insulin and GLP-1 pens from Novo Nordisk, Sanofi, Lilly, and others. You can choose any brand of pen needle regardless of which pen device you use.
What is the difference between 31G and 32G pen needles?
The gauge (G) number indicates the needle’s thickness – the higher the number, the thinner the needle. A 32G needle (0.23 mm diameter) is thinner than a 31G needle (0.25 mm diameter), resulting in slightly less pain during injection. The difference is small but can be noticeable, especially for children or patients who inject multiple times daily.
Can I use pen needles for Ozempic or Wegovy?
Yes. Ozempic and Wegovy pens use the same universal pen needle connection as insulin pens. All pen needles in our collection – BD Ultra-Fine, NovoFine, and BRP Fine – are fully compatible with Ozempic, Wegovy, Trulicity, Victoza, and other GLP-1 receptor agonist pens. A 4 mm or 5 mm needle is typically recommended.
Are pen needles available without a prescription?
Yes, pen needles are classified as medical devices and are freely available without a prescription in Germany. You can order them directly from our online shop without needing a doctor’s prescription. However, patients with a prescription may be able to have the cost covered by their health insurance – check with your provider.
How do I dispose of used pen needles properly?
Used pen needles must be placed in a puncture-resistant sharps container – never in regular household waste. After injection, carefully recap the outer cap using the one-hand scoop technique, unscrew the needle, and drop it into the container. Once full, sealed sharps containers can be returned to most pharmacies in Germany free of charge.
Can I buy pen needles on account (invoice)?
Yes, at Parahealth we offer purchase on invoice (Kauf auf Rechnung) as a payment option for customers in Germany. You receive your pen needles first and pay within the stated payment period. We also offer additional payment methods including PayPal, credit card, and instant bank transfer (Sofortüberweisung).
